IRS Form 990
Private foundations are required to provide a copy of the following documents upon request:
- the three most recent 990-PF's (income tax returns)
- Form 1023 (application for tax exempt status)
- Form 4720 (reporting payment of certain taxes)
These forms are to be provided on the same day for walk-in requests and within thirty days via mail or fax for copies requested by mail.
Walk-in requests for these documents will be received at the Foundation office, 324 West Wendover Avenue, Suite 300, Greensboro, North Carolina, during regular business hours. Requests by mail may be addressed to:
